Annual Program Requirements
Complete the simple steps of the annual program requirements to earn a wellness insurance incentive that affects your bi-weekly premium. The process is simple, you can engage in our annual screenings to complete the initial step of the process. From there you can earn a credit for being a non-smoker and for passing the healthy biometric profile numbers during your screening. If you don't pass the screening, you can still earn the wellness incentive by meeting with your physician for an annual physical. You can also earn additional credit for your spouse/dependent if you submit a proof of physical for them as well.

Individuals can earn 2 credits with an additional credit available for employees with enrolled spouses/dependents

COMPLETE A HEALTH ASSESSMENT
Complete On-Site Biometric Screening or Physical with Primary Care Physician
CREDIT #1 - NON-TOBACCO USER
Earn a credit for a negative continine test during your annual assessment. If you are a tobacco user, you can still earn credit by completing a tobacco cessation program.
CREDIT #2 - BIOMETRIC RESULTS
Pass 3 out of the 4 physical conditions listed below:

Blood Pressure less than 135/90
-
Total Cholesterol/HDL Ratio is less than 5.0
-
Blood Sugar/Glucose less than 115.1
-
Body Fat % level within rage for age
CREDIT #3 - FAMILY COVERAGE
Submit the completed 2024 Spousal Wellness Credit form and submit to The McCahill Group (TMG) via email - forms@mccahillgroup.com
If you are a tobacco user or do not meet 3/4 of the criteria, you can still earn your credits by completing the additional requirement step before the deadline
What if I don't pass the criteria?
If you don't pass 3/4 of the criteria, you must meet with your primary care physican for a physical and submit proof of completion. That will qualify you for the same incentive as having passed the screening.
Tobacco users are also required to participate in a tobacco cessation program to earn their credit of being non-tobacco user
